The Forum Health Assessment

An annual, anonymous survey for peer advisory forums — the classic Forum Survey, sized for eight raters, with a reporting layer that shows what averages hide. Each member answers 19 questions. The forum receives full distributions, not averages, plus a factual operating record from the moderator. The report feeds a facilitated 90-minute debrief where the forum chooses its own actions. Piloting fall 2026.
